Rotherham Active For Health 2016

The Rotherham Active for Health programme aims to support individuals with long term conditions to become and stay more physically active. The programme aims to link NHS clinical rehabilitation services to community physical activity programmes.
The innovative programme includes a 12 week condition-specific (COPD, Cardiac, Falls and Fractures, Stroke, MSK, Cancer) physical activity programme delivered by highly qualified exercise instructors.
Sheffield Hallam University's Centre for Sport and Exercise Science, led by Dr Lindsey Reece, is the proud academic partner for the programme and are conducting an independent evaluation. This video offers a unique insight into how the programme has supported participants during its first year and how physical activity has positively influenced their lives.
