Occupational Health Questionnaire – What You Need to Know
- You’ll receive your questionnaire directly from SOHS via notifications@cority.com.
⚠️ Make sure it’s sent to a personal email (not a school/college one) to avoid access issues.
- You’ll get two emails:
1. Your account details
2. A one-time password
Both will include a link to the questionnaire. You must verify your account before completing it.
- Enter your date of birth in UK format: dd/mm/yyyy
- If needed, a phone or face-to-face appointment will be arranged. Please respond promptly.
- Health course applicants can upload their vaccination history to the questionnaire.
You can request these records from your GP and upload them after submitting the questionnaire.
